Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members Review and document prior and current medical history, existing conditions, and medication history Educate patients on chronic disease management, medication use, and health maintenance Identify potential gaps in care and recommend next steps or interventions as needed Visits may include annual wellness assessments or post-discharge evaluations No prescribing or treatment plan formulation required

Qualifications:
