Job Purpose:
The Licensed Vocational/Practical Nurse responsibility is providing nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence, coordinating care with the Interdisciplinary Group/Team, patient, family, and referring agency.
Duties & Responsibilities:
Under the direction of the RN, assists in identifying the patient's physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations. Participates in planning and implementing care in conjunction with the RN, in accordance with the POC. Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. Assures the continuity of care through delivery of quality patient care. Provides effective communication to the patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes and evaluations. Monitors assigned cases to ensure compliance with requirements of third-party payor. Demonstrates commitment, professional growth, and competency. Promotes the Hospice philosophy and administrative policies to ensure quality of care. Additional duties as assigned. Duties and responsibilities may change, and new ones may be added, at any time with or without advanced notice.
Skills and Qualifications:
Graduate of an accredited School of Vocational/Practical Nursing. State license as a Licensed Vocational/Practical nurse, or a multi-state license issued by a member of the National Nurse Licensure Compact (NLC). Two year experience as a Licensed Vocational/Practical Nurse in a clinical care setting, Hospice experience preferred. Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Current driver's license. Reliable transportation and valid auto liability insurance.
Working Conditions:
Works in patients' place of residence in various conditions. Possible exposure to blood, bodily fluids, and infectious diseases. The ability to work a flexible schedule and extended hours. Ability to travel locally. May experience exposure to unpleasant weather conditions. PRN emergency calls.
Physical & Mental Requirements:
Prolonged standing and walking required. Ability to lift up to 50 lbs and move patients. Requires ability to handle stressful situations in a calm and courteous manner at all times. Requires working under some stressful conditions to meet deadlines and patient needs. Ability to make quick decisions and resource acquisition.
