Key Responsibilities:

Perform annual and post-discharge home health risk assessments, averaging 45 minutes per patient Review patients prior and current medical history, conditions, and medication usage Educate patients on managing chronic conditions and understanding medications Identify gaps in care and recommend appropriate actions to improve health outcomes No requirement for formulating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Texas Board Certified through AANP or ANCC Current Basic Life Support (BLS/CPR) certification Master of Science in Nursing (MSN) from an accredited program A minimum of 6 months recent experience as a nurse practitioner is preferred This position compensates $80-$90 per completed evaluation and includes a mileage stipend, as well as additional compensation for administrative time.
