We have various locations in Central and South Texas in the markets of Austin, San Antonio, Del Rio, Eagle Pass, Harlingen, Laredo, McAllen, Robstown, and Rio Grande City. We are growing and adding positions to our team! The job duties for the position of the Hospice LVN are listed below.
Job Summary:
The LVN is primarily responsible for administering skilled nursing care, under the supervision of a registered nurse, for patients/clients of all ages in their places of residence. Participates in the coordination of coordinate care with the interdisciplinary team, patient/client, family and referring agency.
Supervisory Responsibilities:
N/A Duties/Responsibilities:
  • Works in patients'/clients' homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather
  • Under the direction of the RN, assists in identifying the patient's/client's physical, psychosocial, and environmental needs as evidenced by documentation.
  • Participates in planning and implementing care in conjunction with the RN, in accordance with the POC.
  • Provides patient/client and caregiver education.
  • Provides care using infection control measures that protect both staff and patient/client.
  • Assures the continuity of care through delivery of quality patient/client care.
  • Provides effective communication to patient/client/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner
  • Monitors assigned cases to ensure compliance with requirements of third-party payor.
  • Demonstrates commitment, professional growth, and competency.
  • Promotes Agency philosophy and administrative policies to ensure quality of care. Required Skills
  • Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
  • Proof of current CPR certification Job Qualifications (Education, Experience, Licensure)
  • Graduate of an accredited school of vocational nursing.
Licensure:
Active state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state.
  • Current valid driver's license.
  • One year of experience as a licensed vocational/practical nurse in a clinical care setting required. Hospice experience, required
  • Reliable transportation and valid auto liability insurance.
Fluent in Spanish & English (required)
Physical Requirements:
  • Prolonged standing and walking required, with ability to lift up to fifty pounds and move patients/clients.
  • Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition; meet patient/client and family individualized psychosocial needs.
  • Requires hand-eye coordination and manual dexterity.
