Job Description

Job Summary:
The LVN is primarily responsible for administering skilled nursing care, under the supervision of a registered nurse, for patients/clients of all ages in their places of residence. Participates in the coordination of coordinate care with the interdisciplinary team, patient/client, family and referring agency.Supervisory Responsibilities:
N/A Duties/Responsibilities:
- Works in patients'/clients' homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather
- Under the direction of the RN, assists in identifying the patient's/client's physical, psychosocial, and environmental needs as evidenced by documentation.
- Participates in planning and implementing care in conjunction with the RN, in accordance with the POC.
- Provides patient/client and caregiver education.
- Provides care using infection control measures that protect both staff and patient/client.
- Assures the continuity of care through delivery of quality patient/client care.
- Provides effective communication to patient/client/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner
- Monitors assigned cases to ensure compliance with requirements of third-party payor.
- Demonstrates commitment, professional growth, and competency.
- Promotes Agency philosophy and administrative policies to ensure quality of care. Required Skills
- Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
- Proof of current CPR certification Job Qualifications (Education, Experience, Licensure)
- Graduate of an accredited school of vocational nursing.
Licensure:
Active state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state.- Current valid driver's license.
- One year of experience as a licensed vocational/practical nurse in a clinical care setting required. Hospice experience, required
- Reliable transportation and valid auto liability insurance.
Physical Requirements:
- Prolonged standing and walking required, with ability to lift up to fifty pounds and move patients/clients.
- Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions and resource acquisition; meet patient/client and family individualized psychosocial needs.
- Requires hand-eye coordination and manual dexterity.
