Job Duties and Responsibilities:

Patient Care:

Administer medications, perform wound care, and monitor vital signs. Provide education to patients and their families about managing health conditions and medications.

Assessment and Planning:

Conduct comprehensive assessments of patients' health and develop individualized care plans based on their medical, physical, and emotional needs.

Documentation:

Maintain accurate records of patient progress, care provided, and any changes in condition. Complete necessary paperwork in compliance with agency policies and regulations.

Education and Support:

Educate patients and families on home health care procedures, disease management, and health promotion strategies.

Skills:

Strong clinical skills, excellent communication abilities, and a compassionate approach to patient care are essential.

Minimum Education and Requirements:

Active License to practice: Must hold a current and unencumbered RN license to practice in the state.
