Key Responsibilities:

Perform comprehensive home health risk assessments, typically lasting about 45 minutes each Review and evaluate prior and current medical history, conditions, and medication usage Educate patients on diagnoses and medication management to promote better health outcomes Identify and communicate any gaps in care, recommending actionable next steps Collaborate with care teams as necessary, while avoiding involvement in ongoing treatment planning or prescribing

Qualifications:
