Key Responsibilities:

Conduct in-home annual health risk assessments for Medicare Advantage plan members Collect and review comprehensive medical histories, conditions, and medication use Offer education to patients on their diagnoses and medications Identify gaps in care and propose recommendations (no treatment planning or medication prescribing required) Document evaluations accurately and thoroughly

Qualifications:

Active Nurse Practitioner license in Texas Current CPR (BLS) certification Board Certification via AANP or ANCC At least 6 months of Nurse Practitioner experience preferred MSN degree from an accredited nursing program This role offers a compensation of $80$90 per completed evaluation.
