Job Description

Location:
This position can be based from any Angels of Care branch office in Texas.
Travel:
Occasional travel required. The Skilled Visit Coordinator supports the timely, accurate, and efficient coordination of skilled clinical visits for Angels of Care Pediatric Home Health. This position serves as an important operational connection among clients and families, field clinicians, clinical leadership, and internal support teams to promote continuity of care, appropriate clinician utilization, and timely delivery of authorized services. The Skilled Visit Coordinator is accountable for maintaining accurate schedules and visit information, identifying and resolving scheduling barriers, communicating changes promptly, and escalating concerns that could affect client care or compliance. The role works with a high degree of organization, urgency, and attention to detail while supporting Angels of Care's commitment to exceptional, compassionate care and the HALOS values of Heart, Advocacy, Love, Outreach, and Speed. Duties and Responsibilities Coordinate skilled clinical visits to support timely delivery of ordered and authorized services while promoting continuity of care for clients and families. Maintain accurate clinician schedules, visit assignments, and related information within applicable scheduling, electronic medical record, and operational systems. Partner with clinical leadership and field clinicians to align visit assignments with client needs, clinician availability, qualifications, geographic considerations, and organizational requirements. Monitor upcoming and outstanding visits and proactively address scheduling gaps, cancellations, conflicts, or other barriers that could disrupt continuity of care. Communicate schedule changes and visit-related information promptly and professionally to clinicians, clients, families, and appropriate internal stakeholders. Support effective clinician utilization by identifying scheduling opportunities, balancing operational priorities, and helping minimize avoidable missed or delayed visits. Maintain accurate and timely documentation related to scheduling activity, communications, visit status, and required follow-up in accordance with organizational standards. Identify and escalate unresolved scheduling, staffing, clinical, authorization, or service-delivery concerns to the appropriate leader to support timely resolution and regulatory compliance. Collaborate across clinical, operations, staffing, authorization, intake, and other support functions to facilitate coordinated client service and effective care transitions. Oversee training and competencies for all skilled visits nurses Monitor assigned workflows, reports, and work queues to ensure required actions are completed accurately and within established timelines. Protect confidential client and employee information and perform responsibilities in accordance with HIPAA, applicable healthcare requirements, and Angels of Care policies and procedures. Contribute to process improvement, team coverage, training, and other department initiatives that strengthen service quality, operational efficiency, scalability, and the client and clinician experience.
Qualifications:
Registered Nurse required 3+ years of related professional experience in healthcare coordination, scheduling, operations, customer service, or a comparable environment required Experience supporting home health, pediatric healthcare, skilled nursing, therapy, or another clinical service environment strongly preferred.
