Experienced Nurse Practitioners are sought for per diem opportunities delivering home health risk assessments for Medicare Advantage plan members. This role involves traveling within Burnet County, including areas such as Marble Falls and Burnet, to meet patients in their homes and conduct comprehensive health evaluations.

Schedule:

Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan members Assess medical history, current health conditions, and medications Provide patient education regarding health status, conditions, and medications Identify care gaps and recommend appropriate follow-up actions No responsibility for creating treatment plans or prescribing medications required

Qualifications:

Active Nurse Practitioner license in Texas MSN degree from an accredited nursing school Board certification through AANP or ANCC Current CPR (BLS) certification Minimum of 6 months NP experience is preferred Prior home health experience is advantageous but not required Nurse Practitioners will receive $80$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
