As a vital member of our healthcare team, you will deliver comprehensive nursing services that focus on comfort, dignity, and quality of life. Your expertise in managing complex medical needs, coordinating care plans, and supporting families through difficult times will make a meaningful difference in patients' lives. Responsibilities Conduct thorough patient assessments to evaluate physical, emotional, and psychological needs, ensuring personalized care plans are developed and implemented effectively. Administer medications, manage symptoms, and monitor vital signs to maintain patient comfort and stability. Collaborate with interdisciplinary teams including physicians, social workers, and chaplains to coordinate holistic care approaches. Educate patients and families on disease progression, symptom management, and advanced care planning to empower informed decision-making. Provide emotional support and guidance to patients and their loved ones during challenging moments of end-of-life care. Maintain accurate medical records in compliance with HIPAA regulations, documenting all patient interactions and interventions thoroughly. Participate in discharge planning and post-acute care coordination to ensure seamless transitions across care settings. Skills Critical care experience with a strong background in managing complex medical conditions. Knowledge of hospital medicine practices including patient assessment, vital signs monitoring, and acute care procedures. Familiarity with infectious disease management, post-acute care processes, and outpatient clinic operations. Proficiency in medical terminology, medical records documentation, discharge planning, and utilization management. Strong understanding of physiology principles relevant to palliative and hospice care. Excellent case management skills with the ability to coordinate multidisciplinary teams effectively. Experience with geriatric (Geriatrics), behavioral health, or managed care environments is a plus. Pay:
$80.00 - $150.00 per hour Application Question(s): Have you served as the Hospice Director of Nursing or Clinical Director for a Medicare certified hospice? Have you participated in an initial Medicare survey or state hospice survey? Have you worked for a hospice startup or opening of a new hospice agency?
