Duties Conduct thorough patient assessments focusing on behavioral health, mental status, and physiological conditions specific to geriatric populations Develop and implement personalized care plans that address mental health issues such as depression, anxiety, dementia-related behaviors, and other cognitive disorders Monitor vital signs and physiological indicators to detect changes in patient condition and respond promptly Collaborate closely with physicians, social workers, therapists, and other healthcare professionals to coordinate holistic treatment strategies Manage medication administration and monitor for side effects or adverse reactions in accordance with hospital policies and HIPAA regulations Utilize Electronic Health Records (EHR) systems to document patient progress, medical records, discharge planning details, and clinical case management activities Provide patient education on managing behavioral health conditions, medication adherence, and safety measures within the geriatric context Participate in discharge planning and post-acute care coordination to ensure seamless transition for patients returning home or to long-term care facilities Engage in infection control practices and adhere to health regulation policies to maintain a safe environment for patients and staff Support crisis intervention efforts during behavioral episodes or emergencies in the hospital or outpatient settings Experience Proven nursing experience in geriatrics or behavioral health settings, with a strong foundation in patient service and assessment techniques Critical care experience preferred, especially within Level I or Level II trauma centers or emergency rooms involving complex cases Experience working in hospital medicine, medical-surgical units, or outpatient clinics serving elderly populations is highly desirable Familiarity with ICU or post-acute care environments that involve acute care management and diagnostic evaluation skills Knowledge of infectious disease care protocols relevant to hospital settings and home health environments Proficiency in EMR/EHR systems for accurate documentation of medical records and clinical activities Understanding of HIPAA regulations related to patient privacy and confidentiality in all aspects of nursing practice Strong communication skills for effective client interaction and collaboration with multidisciplinary teams Ability to manage multiple priorities including intake procedures, case management activities, utilization management, and discharge planning Certification as a Geriatric Nurse (GNP) or Behavioral Health RN is preferred; active nursing license required in the state of practice Join us in delivering compassionate, expert care that transforms lives.
