In this vital role, you will deliver comprehensive patient care directly in patients' homes, focusing on recovery, management of chronic conditions, and prevention of hospital readmissions. Your expertise will help bridge the gap between acute care and long-term wellness, ensuring patients receive personalized, high-quality care in their familiar environment. This position offers an exciting opportunity to make a meaningful impact on patient outcomes through proactive health management and collaborative care planning. Duties Conduct thorough clinical assessments including vital signs, physical examinations, and detailed patient histories during home visits. Develop and implement individualized care plans tailored to each patient's needs, including medication management and health coaching. Administer treatments such as IV infusions, botulinum toxin therapy, dermal fillers, or other procedures within your scope of practice. Coordinate with multidisciplinary teams to facilitate discharge planning, case management, and utilization review for optimal patient outcomes. Perform diagnostic evaluations using tools like sonography or laboratory specimen collection/processing when necessary. Educate patients and families on disease management, medication administration, nutrition, and safety protocols to promote independence. Document all patient interactions accurately within electronic health record (EHR) systems such as Epic or eClinicalWorks while adhering to HIPAA regulations. Skills Proven experience in post-acute care settings such as nursing homes, assisted living facilities, or home health environments. Strong knowledge of medical documentation systems including Epic, Cerner, Athenahealth, or similar EMR/EHR platforms. Skilled in performing clinical procedures like suturing, catheterization, phlebotomy, sterile processing techniques, and basic life support. Familiarity with diverse medical specialties including geriatrics, behavioral health, infectious disease care, pain management, and hospice & palliative medicine. Ability to manage complex cases involving acute pain management, ICU experience, trauma center protocols (Level I/II Trauma Center), or emergency medicine procedures. Excellent patient assessment skills with a solid understanding of physiology and anatomy relevant to diverse populations including pediatrics and neonatal ventilator management. Competency in CPT coding, ICD coding (ICD-9/10), and utilization review processes for accurate billing and compliance. Experience working with individuals with disabilities or developmental delays; knowledge of Workers' Compensation law is a plus. Strong communication skills for triage calls and telehealth consultations; ability to educate patients effectively across various age groups including toddlers and infants.
