Schedule:

13-week contract with potential for extension Full-time, Monday through Friday On-call duty may be required Structured daily visit expectations

Key Responsibilities:

Deliver skilled nursing care to patients in their homes, including assessments, interventions, and follow-ups Complete OASIS documentation accurately and in accordance with deadlines Collaborate with physicians and interdisciplinary care teams to develop and implement personalized care plans Perform wound care, administer medications, monitor chronic conditions, and educate patients and families on disease management Communicate changes in patient condition and care updates promptly to the clinical team

Qualifications:

Active RN license Minimum of 1 year of recent experience in home health nursing Proficient in OASIS documentation (required) Familiarity with HomeCare HomeBase (strongly preferred) Organized and able to manage a consistent, full-time schedule with daily patient visits Possesses reliable transportation and valid drivers license
