Job Description
On-Call RN Case Manager (7 On / 7 Off) Peak Home Health & Hospice | Northern Utah Care at the highest level. This isn't just an on-call role—it's a different kind of schedule for a different kind of nurse. At Peak, our on-call RNs are trusted decision-makers. You're the calm in the chaos, the voice families rely on after hours, and the clinician who ensures our patients receive the same high-level care—no matter the time of day. Seven days on. Seven days off. When you're on, you matter. When you're off, you're truly off. What You'll Do Respond to after-hours patient needs, questions, and visits Provide triage, guidance, and in-home support when needed Coordinate with the care team to ensure continuity Deliver confident, compassionate care in urgent moments What We're Looking For Active RN license in Utah Hospice experience strongly preferred Ability to make sound clinical decisions independently Strong communication and critical thinking skills Someone who thrives in autonomy and responsibility Compensation $50 per weeknight on-call $150 per weekend day on-call Additional visit rates paid for each hospice visit completed Why Peak 7-on / 7-off schedule that actually gives you balance Meaningful, high-impact work—not routine tasks A supportive team that has your back Competitive, transparent compensation structure A culture built on trust, accountability, and excellence
