Job Description
REGISTERED NURSE--HOME HEALTH/HOSPICE ON CALL EVENINGS & WEEKENDS

This position would provide on call support rotating weeknights and weekends.ON CALL NURSE ESSENTIAL FUNCTIONS
- Takes call from Friday 5:00pm to Monday 8:30am two weekends per month.
- Takes call rotating weekday evenings from 5:00pm to 8:30am.
- Field/triage patient/family calls and concerns
- Conducts routine visits and/or admit visits with home health or hospice patients
- Assures thorough team communication regarding patient status at shift end
- Provides community education to potential patients and their families
ON CALL RN QUALIFICATIONS
- Cheerful customer service oriented.
- Current Utah RN license (required).
- Experience in the home health and hospice settings (2 years RN experience preferred)
- Excellent availability and response time to calls.
- Demonstrated ability to triage and apply critical thinking skills.
- Knowledge of and ability to navigate electronic medical records. HomeCare HomeBase experience a plus.
- Reliable transportation is required.
