Job Requirements:
Initial Visit to assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care. Chart all clinical documentation in our EHR software Create a patient plan of care & Coordinate services Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan. Provides medication education and management per physician order Works hand-in-hand with the IDT and IDG to ensure patient goals are met and appropriate outcomes are achieved Shares on-call duties Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams
Job Qualifications:
Registered nurse with current license to practice professional nursing in the state of Utah Polite and compassionate Excellent observation, problem solving, verbal and written communications Maintains a current CPR certification Must be a licensed driver with an automobile that is insured, registered and in good working order Good communication and smartphone skills Ability to pass a background check
