We are looking for a Home Health/Hospice RN Case Manager who is passionate about providing the best care for our patients Experience preferred but will train the right person.

Salt Lake, Utah (Area)
RN Case Manager Requirements:
Self-motivated and dependable Strong clinical skills using critical thinking and reasoning Compassionate and caring Detail and deadline oriented High ethical standards Works well with others and as a team member Excellent verbal and written communication skills Computer skills Reliable transportation
RN Case Manager Responsibilities:
Coordinate the overall care of the patient while interacting with patient, family, physicians, Interdisciplinary Team members and other healthcare professionals. Provide professional nursing care for both home health and hospice patients in their place of residence. Complete initial, comprehensive and ongoing assessments for patients to determine their needs. Conduct nursing visits for pain and symptom management, wound care, medication administration and other skilled care as ordered by the physician. Complete all documentation and charting accurately and in a timely manner. Monitor and supervise those involved ensuring that the patient receives the best possible care.
Schedule:
Full-Time flexible; Monday- Friday expected; 8-5 flexible On-call not required, unless desired (we have a seperate on-call team)
Pay:
DOE Licenses Required:
