Responsibilities 1.Administer prescribed medications and treatments as prescribed and according to the laws of scope of practice 2. initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living 3. Development, revision and Implementation of the individualized plan of care and recommending revisions to the plan as necessary with the partnership of the patient, representative if any and caregiver. 4. Provide consultation and continuing education to the patient/family regarding patient's condition, medications, function, decline, diagnosis, services 5. Prepare clinical and progress notes that demonstrate progress towards established goals 6. Prepare clinical and progress notes that support the patients terminal diagnosis and decline (Hospice) 7. Coordinate all patient/family services with prioritization of needs with the members of the interdisciplinary team 8. Use of case management approach and referring to other services as needed 9. Inform the physician and other personnel of changes in patient's needs and outcomes of intervention 10. Provide services that are ordered by the physician as indicated in the plan of care 11. Provide counseling for the patient, caregiver and family 12. Provide patient and caregiver education 13. Determine the scope and frequency of services needed based on acuity and patient/family needs 14. Provide supervision of LPN'S/LVN'S and paraprofessionals providing services to the patient according to regulatory guidelines 15. Participate in in service programs 16. On-going evaluation of patient/family response to care 17. Assess the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care 18. Evaluate own need for support and using identified systems to meet the needs 19. Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility 20. Communicate information using current process and technology available to the organization 21. Participate in the hospice performance improvement program 22. Assess risk for pathological grief 23. Assess for cultural and spiritual implications 24. Assess for verbal and non-verbal communication patterns 25. Manage discomfort providing symptom relief 26. Attend training and educational events, case conferences to continually work on specialized nursing skills related to palliative and end-of-life care. This is to include the disease process, self-care techniques, end-of-life care, process for dealing with issues of ethical concern 27. Nursing services include seeing, treating and writing orders for hospice patients in accordance with state laws 28. Provide care ordered and written in accordance with the plan of care 29. Complete initial and on-going assessments and care plans on all patients assigned. This includes preparing IDT notes q14 days prior to meeting that reflect on-going eligibility 30. Coordinate with physician to ensure completion of face to face visits on 60 day re-certifications 31. Write a clinical or progress note for all contact with patient including actual visits or phone communications and record in patient's record 32. Teach patient and caregiver self-care techniques 33. Participation in the Home Health and Hospice QAPI programs 34. Case Management of patient's assigned. Re-evaluate the patient with any change in condition and notify physician as well as IDT members of changes 35. Work under the direction of the hospice physician in regards to all changes in patient status and needs 36. Provide communication with all physician's involved in the plan of care and other health care practitioners as appropriate related to the current plan of care 37. Order equipment and medications patients need ensuring medications are filled before patient runs out 38. Assist DON/clinical manager in monitoring quality assurance areas, providing input in measures being monitored as well as measures that would be beneficial to monitor 39. Provide patient care as requested per patient's needs and directed by supervisor 40. Assist Social Worker and Chaplain in providing support to patient and/or family members 41. Complete shared visits when needed 42. Write orders for any changes to plan/level of care 43. Inform office staff when changes such as respite, GIP etc. take place. Nurse is responsible for assuring family understands medication needs etc. for these instances 44. Write any verbal orders in homecare and update the care plan as necessary 45. Attend required in-services 46. Provide services in accordance with organizational policies and procedures and regulations that define scope of practice Qualifications 1. Must have a valid license in the State of Utah 2. Graduated from an approved school of professional nursing (Sec.484.4) 3. Must have own transportation 4. Must pass a background check. 5. Must have two years of nursing experience if supervisory is necessary.
