Woods Cross, UT Job Details Full-time 5 hours ago Benefits Health insurance Dental insurance 401(k) Paid time off Vision insurance Life insurance Qualifications Customer communication Computer literacy Nursing Full Job Description Rocky Mountain Care is looking for a full-time Intake/Admissions Nurse (RN/LPN) Mon-Friday 9a-6p, One weekend every 5-7 weeks. Rotating on call every 5-6 weeks. Responsibilities and Duties Facilitate admissions for incoming Skilled Nursing patients (does not include direct patient care) Verify and enter physician orders Identify level of care needed Receive and give verbal orders accurately Review clinical documentation to determine Skilled Nursing placement Exceptional interpersonal and customer service skills Ability to communicate clearly with medical providers Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families Manage after-hours and weekend referrals (rotating on call)
Qualifications:
Current license as a licensed LPN/RN from the Utah State Board of Nursing 1-year Skilled nursing facility experience preferred Good verbal and written communication skills Basic computer knowledge Ability to effectively problem solve Excellent customer service skills Knowledge of Medicare/Medicaid Skilled Nursing placement criteria preferred Schedule Monday - Friday 9am-6p One schedule weekend every 5-7 weeks Rotating on-call approximately every 5-6 weeks
