Schedule 7 Night On 7 Nights Off Full-Time Benefits Manageable Caseload Supportive Clinical Team What You'll Do As a Hospice RN Case Manager, you will: Coordinate and manage the care of hospice patients throughout their end-of-life journey Perform comprehensive nursing assessments Develop and update individualized plans of care Provide skilled nursing visits and symptom management Educate patients and families regarding disease progression and comfort measures Collaborate with physicians, social workers, chaplains, aides, and other interdisciplinary team members Participate in after hour on-call coverage during scheduled work week Ensure accurate and timely clinical documentation Qualifications Active Registered Nurse (RN) License Minimum one year of nursing experience required Hospice or Home Health experience preferred Current CPR Certification Valid Driver's License and Reliable Transportation Strong assessment, communication, and time-management skills Why Join Nayar Hospice?
