Warm Hearth at Home Join an Award-Winning Home Health Team Voted Best of Virginia 2026 Warm Hearth at Home is seeking a compassionate and motivated Registered Nurse (RN) to provide skilled home health services throughout Floyd County, Riner, Pilot, and Christiansburg. Our team is dedicated to helping patients remain safely and comfortably in their homes while receiving high-quality care. Position Summary The Home Health RN is responsible for conducting patient assessments, developing and managing individualized care plans, providing skilled nursing services, coordinating care with physicians and interdisciplinary team members, and educating patients and caregivers. Responsibilities Complete Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge assessments Provide skilled nursing visits in patients' homes Develop and update individualized plans of care Monitor patient progress and communicate changes to physicians Educate patients and caregivers on disease management, medications, and safety Coordinate care with therapy, social work, hospice, and other healthcare professionals Maintain accurate and timely clinical documentation Participate in on-call rotation as assigned Qualifications Current Virginia Registered Nurse (RN) license Minimum one year of nursing experience preferred Home health experience preferred but not required Strong assessment, communication, and organizational skills Valid driver's license and reliable transportation Ability to work independently while collaborating with a supportive team Benefits Competitive salaried compensation Medical, dental, and vision insurance Paid time off Mileage reimbursement at 0.65 cents per mile Continuing education opportunities Supportive leadership team Flexible scheduling Complimentary membership to the Warm Hearth fitness center/gym One complimentary meal each workday (up to 5 meals per week) from our Assisted Living dining services 401(k) retirement plan with a 4% company match after one year of employment when you contribute at least 5% Why Warm Hearth at Home?
