Job Description

Overview 1st Option Home Health, LLC is seeking an experienced and highly organized Registered Nurse (RN) with WaMS and Virginia Medicaid Waiver experience to provide clinical oversight for our home and community-based services. This position combines clinical nursing oversight, WaMS management, care coordination, documentation review, staff support, and regulatory compliance. The ideal candidate understands Virginia's DD Waiver system and can independently help ensure that services, clinical records, plans of care, physician orders, and service authorizations remain accurate, current, and compliant. This is an excellent opportunity for an RN who enjoys the clinical and administrative side of nursing and wants to play a key role in maintaining quality and compliance as our organization grows. Responsibilities Provide ongoing RN clinical oversight for individuals receiving home and community-based services Complete and review nursing assessments, reassessments, and Plans of Care Review physician orders, medication records, diagnoses, and other clinical documentation Monitor changes in individuals' health status and coordinate appropriate clinical follow-up Review and manage information within the Virginia Waiver Management System (WaMS) Monitor service authorizations, approved units, authorization periods, renewals, and supporting documentation Review Individual Support Plans (ISPs) and ensure clinical services align with identified needs and authorized services Coordinate with Support Coordinators, physicians, pharmacies, families, and other members of the interdisciplinary team Provide clinical guidance and oversight to LPNs and direct care staff Identify staff training and competency needs based on individual clinical requirements Review clinical documentation for accuracy, completeness, and regulatory compliance Conduct chart reviews and assist with quality assurance activities Review incidents requiring nursing assessment or clinical follow-up Identify documentation, authorization, or clinical concerns and follow them through resolution Assist with admissions, discharges, hospital transitions, and changes in level of care Participate in interdisciplinary meetings and care conferences as needed Support preparation for DMAS, DBHDS, Medicaid, and other regulatory audits or reviews Help develop and maintain strong clinical systems, policies, and quality-improvement processes Qualifications Required Current, unrestricted Virginia Registered Nurse (RN) license Strong clinical assessment and documentation skills Excellent organizational, communication, and time-management skills Ability to work independently and manage multiple clinical priorities Proficiency with electronic documentation systems and online healthcare portals Strongly Preferred Direct experience using WaMS (Waiver Management System) Experience with Virginia Medicaid DD Waivers Knowledge of Community Living (CL), Family & Individual Supports (FIS), and Building Independence (BI) Waivers Experience working with individuals with intellectual and developmental disabilities Home health, HCBS, private duty nursing, or community-based nursing experience Familiarity with DMAS and DBHDS requirements Experience with ISPs, service authorizations, Plans of Care, clinical audits, and Medicaid documentation Nursing supervisory or clinical oversight experience BSN preferred Ideal Candidate We are looking for more than an RN who can provide direct patient care. The ideal candidate understands how WaMS, service authorizations, ISPs, Plans of Care, physician orders, nursing documentation, staff competency, and Medicaid compliance work together . You should be comfortable reviewing a case, identifying what is missing or needs correction, communicating with the appropriate team members, and following through until the issue is resolved.
