Full Time, Part Time To be eligible, applicants must reside in Northern Virginia, Position Summary We are seeking a compassionate, experienced Registered Nurse (RN) Care Manager to join our team. The RN Care Manager is responsible for assessing, planning, coordinating, and advocating for the care needs of our clients—primarily aging adults and those with complex medical or psychosocial conditions. This role focuses on client-centered care management, family support, and collaboration with healthcare providers to ensure optimal outcomes and quality of life.

ESSENTIAL JOB FUNCTIONS

The successful candidate must be willing, helpful, supportive, cooperative, accountable, have attention to detail and be able to make decisions based on established guidelines and procedures. The essential functions of this position are people focused and helpful in nature; providing support and/or service to others. Care managers will work with adult/aging clients who need unbiased and professional assistance. This includes guidance related to healthcare, aging, household & financial tasks. Responsibilities include but are not limited to: Assess and evaluate and assess older adults and adults living with chronic illnesses. Conduct comprehensive assessments of clients in home or facility settings. Transports clients to medical appointments and outside outings. Personal vehicle is required and utilized. Coordinate medical appointments, home care services, transitions of care, and community resources. Develops comprehensive care plans to address client needs and goals. Monitor ongoing health status, medication management, and adherence to care plans. Serve as a liaison between clients, families, healthcare providers, and other professionals. Provide education to clients and caregivers about chronic conditions, medications, and health management. Conducts routine home and community visits. Advocates for clients. Opportunity to make a meaningful impact in the lives of clients and families. Provide education to clients/families about options for support resources. Attend bimonthly in person staff meetings and company trainings as required.

On-Call Coverage:

Able and willing to participate in a rotating after-hours on-call schedule.

EDUCATION, EXPERIENCE, AND REQUIREMENTS

• Active Registered Nurse (RN) license in Virginia, in good standing.
• Minimum of 3 years of clinical nursing experience ; geriatrics, home health, case management, or care coordination experience preferred.
• Experience supporting older adults or adults with chronic illnesses, including their families or caregivers, preferred.
• Care management experience with older adults or adults with disabilities preferred.
• Experience working with individuals living with dementia preferred.
• Working knowledge of community resources, entitlement programs, and private insurance systems.
• Reliable vehicle, valid driver's license, and safe driving record required.
• Proficiency with Microsoft Office Suite, including Excel, Word, Outlook, and PowerPoint.
• Availability and willingness to participate in the on-call rotation. Qualifications
• Knowledge of community resources, Medicare/Medicaid, and long-term care systems.
• Experience in Aging Life Care or geriatric care management preferred.
• Strong critical thinking, organization, and communication skills.
• Ability to work independently, manage multiple clients, and navigate complex care systems.
• Proficient with electronic documentation and technology, including Microsoft Office and EHR systems.
• Valid driver's license, reliable transportation, and willingness to travel locally.
• Ability to provide after-hours and weekend on-call coverage for client emergencies every 6-8 weeks.
• Candidates must currently reside in Fairfax County, Arlington County, or Alexandria City and be familiar with the local area.

JOB SKILLS & ABILITIES INCLUDE

Excellent written and verbal communication skills with attention to detail. Ability and willingness to travel throughout the DMV area during work hours as dictated by position needs. Ability to work independently and collaboratively with interdisciplinary teams. Proficiency with technology, including use of smartphone and web-based office systems. Ability to work independently, organize travel efficiently, manage multiple projects in a fast-paced environment. Ability to pass a background screen.

MENTAL AND PHYSICAL REQUIREMENTS

The team member regularly is required to stand, walk, sit, bend and reach with hands and arms above, below and at shoulder height. The team member frequently lifts and/or moves up to 25-50 pounds. Must be able to push a wheelchair a minimum of 250 pounds and be able to push such weight a minimum distance of 300 feet without stopping. Work area may be subject to noise, distractions, and interruptions. Ability to remain calm and positive in stressful situations is required.
