Collaborate with residents and healthcare team to deliver compassionate hospice and palliative care Assist with patient assessments and develop care plans in coordination with physicians and staff Provide support and guidance to patients and their families Document patient care activities accurately and timely Participate in team meetings and contribute to quality improvement initiatives Follow facility protocols and regulatory requirementsWhat You Need:
Active Medical License in Virginia or eligible for license reciprocity Prior experience in hospice and palliative medicine (preferred) Strong communication and compassionate care skills Ability to work independently and as part of a collaborative team About theAssignment:
