What You'll Do:

Collaborate with residents and healthcare team to deliver compassionate hospice and palliative care Assist with patient assessments and develop care plans in coordination with physicians and staff Provide support and guidance to patients and their families Document patient care activities accurately and timely Participate in team meetings and contribute to quality improvement initiatives Follow facility protocols and regulatory requirements

What You Need:
