We provide compassionate care for elderly clients, cancer patients, individuals with disabilities, and those in need of respite or companionship services. Job Overview We are seeking a skilled and compassionate Home Health Care Registered Nurse to deliver high-quality nursing care in patients' homes. The ideal candidate will possess extensive clinical expertise, excellent patient assessment skills, and a strong commitment to improving patient outcomes. This role offers an opportunity to make a meaningful difference in the lives of diverse clients through personalized care and professional support. Responsibilities Conduct thorough patient assessments to develop individualized care plans that address medical needs and personal preferences. Administer medications and fill pill boxes. Monitor vital signs, perform physical examinations, and evaluate patient progress during home visits. Provide education on disease management, medication administration, and health maintenance tailored to each patient's condition. Collaborate with physicians, case managers, and other healthcare professionals to coordinate comprehensive care delivery. Document all patient interactions. Requirements Valid nursing license with current registration in the state of practice. Proven experience in home health care nursing or related settings such as hospice, long-term care, or hospital medicine. Knowledge of medical terminology, anatomy, physiology, and infection control practices. Experience working with individuals with disabilities or developmental delays is highly desirable. Strong communication skills coupled with a compassionate approach to patient-centered care. From $117,000.00 per year
