Duration:

13-week contract with potential for extension

Pay Range:

$2350/week gross Responsibilities Provide skilled nursing care to patients in their homes in accordance with physician orders and care plans Perform patient assessments and monitor health status changes Administer medications, treatments, and wound care as prescribed Accurately document visits, assessments, and interventions in Homecare Homebase (HCHB) Educate patients and families regarding treatment plans, medications, and disease management Coordinate care with physicians, therapists, case managers, and interdisciplinary team members Ensure compliance with infection control standards and agency policies Maintain timely communication with clinical staff regarding patient progress and concerns Support patient independence, safety, and overall well-being in the home setting Requirements Active RN license Minimum 2 years of Home Health RN experience required Homecare Homebase (HCHB) experience required Current BLS certification Strong clinical assessment and documentation skills Excellent communication and time management abilities Required Documents to Submit Your Profile To begin the onboarding and profile submission process, please provide: Updated Resume RN License BLS Certification Driver's License Skills Checklist Two Professional References Vaccination Records and Required Medical Documents Benefits Referral program
