Join a contract/travel opportunity in Barre, VT as a Registered Nurse providing hospice case management. This role starts ASAP, follows a Monday-Friday schedule (36 hours/week), and offers $2,308 per 36-hour week.

Requirements:

Active RN license in Vermont. 1-2+ years of clinical nursing experience, ideally in hospice, palliative care, or home health. Strong patient assessment, clinical judgment, and communication skills. Working knowledge of hospice regulations, standards, and individualized care planning.

Key Responsibilities:

Conduct comprehensive assessments and develop, implement, and evaluate individualized hospice care plans. Coordinate care across interdisciplinary teams, including physicians, nurses, social workers, and other providers. Educate and support patients and families on disease progression, symptom management, and end-of-life decision-making. Maintain accurate, timely documentation in accordance with hospice standards and regulatory requirements. Monitor patient status and revise care plans as needed to ensure optimal comfort and quality of life. Participate in interdisciplinary team meetings and contribute to quality improvement efforts.
