Responsibilities:
Provide direct patient care, including administering medications, performing vital signs, and monitoring patients' conditions Develop and implement individualized care plans in collaboration with the interdisciplinary team Identify and address patient needs, concerns, and preferences Communicate effectively with patients, families, and healthcare team members Participate in quality improvement initiatives and educational programs Collaborate with other departments to ensure seamless care coordination
Requirements:
RN or LPN licensure in the state of Vermont Strong clinical skills and knowledge of nursing practices Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment
