Pay Package Weekly Gross Pay:
$2680 We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will assess patient needs, develop and implement individualized care plans, provide treatments, monitor patient progress, and coordinate care with physicians and other healthcare professionals.
Key Responsibilities:
- Provide skilled nursing care to patients in their homes.
- Perform comprehensive patient assessments and develop individualized care plans.
- Administer medications, treatments, and other prescribed interventions.
- Monitor patients' conditions and report changes to the appropriate healthcare team.
- Educate patients and their families regarding medications, treatment plans, and home care.
- Maintain accurate and timely patient documentation.
- Coordinate patient care with physicians, therapists, and other healthcare professionals.
- Follow agency policies, clinical standards, and safety guidelines.
- Work independently while maintaining effective communication with the care team.
Job Requirements:
- Active Registered Nurse (RN) license in Vermont and New Hampshire , or an active, unencumbered Compact RN License .
- Graduate of an accredited school of nursing.
- Minimum 2 years of recent Home Health experience .
- Active AHA BLS Certification required.
COVID-19
vaccination required.
- Valid Driver's License (DL) required.
- 2 professional references , preferably from a manager or supervisor.
- Strong clinical, communication, documentation, and patient-care skills.
- Ability to work independently and travel to patient homes as required.

$2,650.00 - $2,680.00 per week
