The University of Vermont Health Network is hiring a Registered Nurse at Helen Porter Rehabilitation and Skilled Nursing. In this role, you'll deliver compassionate, evidence-based care to post-acute and long-term residents, performing assessments, administering medications, and managing complex care needs. You will develop and update individualized care plans, support rehabilitation goals, and educate patients and families. Working within a collaborative, academic health system, you'll partner closely with therapists, physicians, and support staff while advancing your skills through ongoing learning and professional development. Responsibilities Deliver direct nursing care to post-acute and long-term residents, including assessments, treatments, and evaluations. Administer medications and therapies safely, monitoring for effectiveness and adverse effects. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team. Support rehabilitation and restorative goals, coordinating closely with PT/OT and other therapists. Document care accurately and timely in the electronic health record in accordance with regulatory standards. Educate patients and families on conditions, medications, discharge planning, and self-care strategies. Follow infection prevention, safety, and quality protocols to ensure a safe care environment. Serve as a clinical resource and mentor for LPNs, LNAs, and support staff on the unit. Participate in care conferences, rounds, and quality improvement initiatives.
