All-Inclusive Guaranteed:
36 hours/week Position Overview We are hiring an experienced RN Case Manager - Home Health to provide nursing care to patients in their homes. The RN will work independently while collaborating with physicians, patients, families, and an interdisciplinary care team. Key Responsibilities Assess and monitor patients in the home setting. Develop and manage individualized patient care plans. Coordinate care with physicians and interdisciplinary team members. Monitor changes in patients' clinical and functional conditions. Educate patients and caregivers on medications, diet, treatment, and self-care. Maintain accurate patient documentation. Participate in case conferences and discharge planning. Communicate patient progress and significant changes to the healthcare team. Qualifications Active
VT/NH RN
license or unencumbered Compact RN license. Graduate of an accredited nursing program. Minimum 2 years of Home Health experience . Current
BLS/BCLS
certification . Strong communication, assessment, and organizational skills. Ability to work independently and provide compassionate patient care. Compensation & Benefits $75/hour All-Inclusive Overtime after 40 hours/week: +$10/hour Holiday rate: +$10/hour Mileage reimbursement: $0.61/mile or current VNH rate $5/hour pager rate for on-call Facility-provided computer, cell phone, and nursing equipment
