Elevate patient care in the community by performing focused home health risk assessments as an experienced Nurse Practitioner. This per diem position offers the opportunity to serve a diverse Medicare Advantage population throughout Whatcom County, including Bellingham, Ferndale, Lynden, and Everson. Assessments typically last 45 minutes, ranging from annual health evaluations to post-discharge follow-ups, and contribute to improved health outcomes without the responsibility of prescribing or developing treatment plans.

Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review and document prior/current medical histories, existing conditions, and medication regimens Educate patients on their diagnoses, management strategies, and medication safety Identify and communicate gaps in care, providing actionable recommendations Focus solely on assessment and educationcreation of treatment plans and prescribing are not required

Qualifications:

Active Nurse Practitioner license in Washington State Current CPR/BLS certification Board-certified through AANP or ANCC Associate at least 6 months of recent Nurse Practitioner experience (preferred) Master of Science in Nursing from an accredited institution
