Essential Job Functions/Responsibilities:
Patient Care:
Completes comprehensive assessments of patient and family to determine hospice needs Provides professional nursing care utilizing the nursing process Initiates and revises plan of care as patient status and needs change
Communication:
Maintains accurate and relevant clinical notes regarding patient's condition and care given Communicates with physicians, community health related persons, and families/caregivers Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency
Additional Duties:
Participates in on-call duties as defined by the on-call policy Supervises ancillary personnel and delegates responsibilities when required Fulfills the obligation of requested and/or accepted case assignments
Position Qualifications:
Graduate of an accredited school of nursing Registered Nurse, with a License in good standing to practice Minimum of one (1) year of experience as a professional nurse within the last three (3) years or a baccalaureate degree in nursing and minimum of two (2) years' experience Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, and nursing skills per competency checklist Ability to lift, position, and/or transfer patients, and to lift supplies and equipment Possesses and maintains CPR certification
