Guided by the West African principle of Sankofa, our clinic delivers holistic, culturally sensitive care that respects each patient's history, culture, and lived experience. The Family Practice Nurse will play a vital role in providing preventive care, chronic disease management, wellness visits, weight management, and family medicine services across the lifespan through both in-person and telehealth appointments. This role involves direct patient interaction, client communication, and collaboration with healthcare providers to ensure comprehensive, personalized care. Duties Provide high-quality nursing care to patients across all age groups in a family practice setting. Conduct patient assessments, including health histories and physical examinations. Educate patients on health management, preventive measures, and treatment plans tailored to their individual needs. Assist with chronic disease management and wellness visits to promote overall health and well-being. Support weight management programs and other lifestyle interventions. Communicate effectively with clients to address questions, provide guidance, and ensure understanding of treatment plans. Collaborate with physicians and other healthcare team members to develop and implement patient care plans. Document patient interactions accurately in electronic health records while maintaining confidentiality. Participate in telehealth appointments as needed, ensuring seamless virtual patient care delivery. Stay informed about current nursing practices and healthcare regulations to maintain compliance and deliver evidence-based care. Qualifications Valid nursing license (RN or equivalent) in the relevant jurisdiction. Proven experience in family practice or primary care settings is preferred. Strong communication skills with an emphasis on client interaction and education. Ability to work collaboratively within a multidisciplinary healthcare team.
