Founded by Dr. Joycelyn Thomas, our clinic emphasizes whole person care that respects each patient's history, culture, and lived experience, guided by the West African principle of Sankofa. The Registered Nurse will play a vital role in delivering comprehensive healthcare services through both in-person and telehealth appointments, focusing on preventive care, chronic disease management, wellness visits, weight management, and family medicine across the lifespan. This role involves collaborating closely with healthcare providers to ensure high-quality patient care and utilizing electronic medical records (EMR/EHR) systems efficiently. Responsibilities Provide direct patient care by assessing health status, documenting findings accurately within EMR/EHR systems, and developing individualized care plans. Assist with preventive health services, chronic disease management, wellness visits, weight management programs, and family medicine consultations. Support patients through education on health conditions, medication adherence, lifestyle modifications, and wellness strategies. Collaborate with physicians and other healthcare team members to coordinate patient care effectively. Utilize EMR/EHR systems to document patient interactions thoroughly and maintain accurate medical records. Monitor patient progress and follow up on treatment plans to ensure optimal health outcomes. Adhere to all clinical protocols, safety standards, and privacy regulations to ensure a safe environment for patients and staff. Requirements Valid Registered Nurse (RN) license in the relevant state or jurisdiction. Proven experience working as an RN in a primary care or outpatient setting preferred. Proficiency in using EMR/EHR systems for documentation and patient management. Strong communication skills with the ability to provide compassionate patient education. Ability to work collaboratively within a multidisciplinary healthcare team. Knowledge of preventive care practices, chronic disease management, and family medicine principles is desirable. Commitment to delivering culturally sensitive and relationship-centered care aligned with The Sankofa Practice's mission.
