Key Responsibilities:

Perform in-home annual health risk assessments for Medicare Advantage members in Spokane County, including Spokane Valley and Opportunity Review prior and current medical history, assess medical conditions, and discuss medication regimens Deliver patient education regarding ongoing conditions and prescribed medications Identify care gaps and recommend appropriate next steps for resolution Focus on assessment; no prescribing or treatment plans required

Qualifications:

Active Nurse Practitioner license in Washington State Current CPR (BLS) certification Board Certification through AANP or ANCC Minimum of 6 months experience as a Nurse Practitioner preferred MSN degree from an accredited nursing program This role provides a highly competitive pay structure with compensation of $80$90 per evaluation, including a mileage stipend and additional compensation for administrative time.
