This role involves conducting in-home risk assessments for Medicare Advantage plan members across communities including Spokane Valley and Opportunity.

Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members Evaluate prior/current medical history, ongoing conditions, and medication use Provide patient education regarding conditions and prescriptions Identify gaps in care, make recommendations to address them Each assessment typically lasts approximately 45 minutes and may include annual or post-discharge visits No prescribing, ordering, or treatment plan creation required

Qualifications:

Active Nurse Practitioner license in Washington Board Certification through AANP or ANCC Valid CPR certification (BLS) MSN degree from an accredited nursing program Minimum of 6 months NP experience (preferred)
