Key Responsibilities:

Complete in-home health risk assessments, averaging 45 minutes per visit Assess and review prior and current medical histories, chronic conditions, and medication usage Educate patients on disease management and medication adherence Identify gaps in care, providing recommendations to improve patient outcomes Coverage area includes Bellingham and surrounding communities within Whatcom County No requirement to create ongoing treatment plans or prescribe medicines

Qualifications:

Active Nurse Practitioner license in Washington State Current BLS/CPR certification Board Certification through AANP or

ANCC MSN
