Key Responsibilities:

Conduct home-based risk assessments, typically lasting about 45 minutes per visit, in locations such as Ferndale, Lynden, Everson, Bellingham, and other areas within Whatcom County Evaluate patients prior and current medical history, medical conditions, and medication use Educate members on their medical conditions and prescribed medications Identify care gaps and provide actionable recommendations to close those gaps Strictly no treatment plan development or medication prescribing involved

Qualifications:

Active Nurse Practitioner license in Washington State MSN degree from an accredited school of nursing Board certification through AANP or ANCC Current Basic Life Support (BLS) CPR certification Minimum of 6 months recent NP experience preferred This role compensates $80$90 per evaluation, including mileage stipend and administrative time.
