Schedule & Hours:

Monday Friday, 8:00 AM 5:00 PM Key responsibilities include: Conducting thorough OASIS assessments and developing personalized care plans. Providing direct nursing care such as wound care, IV therapy, medication administration, and chronic disease management. Monitoring and evaluating patient condition, reporting any significant changes to the broader care team and physicians. Educating patients and families regarding diagnoses, medications, treatment options, and healthy lifestyle choices. Maintaining accurate, timely documentation in Home Care Home Base (HCHB). Collaborating closely with the interdisciplinary team, including therapists and social workers, to achieve optimal patient outcomes. Participating in case conferences and ongoing training sessions. Upholding regulatory and care standards in every aspect of delivery.

Ideal candidates will possess:

An active, unrestricted RN license in the state of practice. At least one year of recent field-based Home Health experience. Proficiency in Home Care Home Base (HCHB) and strong OASIS skills. Reliable transportation, a valid drivers license, and current auto insurance.
