ABOUT THE ROLE
The Care Team Lead role is critical to our mission and sets the 2nd Family experience apart. CareTeam Leads apply strong communication, clinical expertise, problem-solving, and coaching skills to keep the relationship between caregivers (NACs), clients, and their loved ones running smoothly and ensure we deliver with excellence. As part of the Flagship Care Team , you'll be directly involved in bringing the 2nd Family mission to life in the greater Seattle area and Eastside. Your skills, experience, and leadership will directly shape our approach and serve as the model to our growing team. There will only be one Flagship Care Team - get on board !

BASELINE CARE TEAM LEAD RESPONSIBILITIES
Conduct initial in-home consultations, including assessments, and document care plans Reassess clients every 90 days (or with change of condition) and update care plans Orient assigned caregivers (NACs) to care plans and assess their skills Serve as primary point of contact for clients and their families when questions or concerns arise Communicate with and support caregivers (NACs), providing direction and mentorship as needed Review caregiver notes for each shift and follow up as needed Be available to take calls or respond to emails during on-duty hours (7 am-7 pm, unless otherwise agreed) Respond to occasional emergency communication during off-duty hours Conduct initial consultations or change-of-condition assessments within 24-48 hours of request Attend monthly company meetings (virtually or in-person) Occasionally support new employee orientations and in-service training
SCHEDULE
The Care Team Lead role is primarily remote and day-to-day responsibilities are addressed as needed or on a self-directed basis. Exceptions include conducting in-person assessments to build out your client base, and participating in occasional training or team meetings.
COMPENSATION STRUCTURE
✓ Initial assessment: $150 + mileage ✓ 90-day reassessment: $75 + mileage ✓ Active client management: $50 per client per week ✓ Orientation, in-service, or other support: fees vary ✓ Weekly pay with direct deposit ✓ Performance-based incentives ✓ Free online continuing education courses
WHAT WE'RE LOOKING FOR
Active RN license in Washington State Home care, community health, or similar experience Coaching, mentorship, or supervisory experience Strong documentation and communication skills Reliable transportation for in-person assessments Flexibility and leadership presence during the launch phase A collaborative, solutions-oriented mindset and genuine passion for the people we serve
