Seeking an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis. This role focuses on evaluating Medicare Advantage plan members in Pacific County, WA. Flexible scheduling is available, allowing you to create your own shifts between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment. Locations may include Raymond, South Bend, Long Beach, and neighboring areasassignments may vary by shift.

Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members Review and evaluate prior and current medical history and conditions, including medication history Provide patient education regarding medical conditions and prescribed medications Identify gaps in care and suggest actionable recommendations to promote optimal health No requirement for treatment plan development or medication prescribing

Qualifications:

Active Nurse Practitioner license in Washington Current CPR certification (BLS required) Board certification through AANP or ANCC Masters degree in nursing (MSN) from an accredited program At least 6 months of recent NP experience preferred Competitive compensation includes $80 to $90 per evaluation, plus a mileage stipend and payment for administrative time.
