Key Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members Assess patients prior and current medical history, medical conditions, and medication use Educate patients on managing medical conditions and medications appropriately Identify and address gaps in care, providing actionable recommendations Visits typically last approximately 45 minutes, varying between annual evaluations and post-discharge assessments No treatment planning or prescribing required; this is a purely assessment-based role

Qualifications:

Active Nurse Practitioner license in Washington State Board Certification through AANP or ANCC Valid and current CPR (BLS) certification 6 months or more of recent nurse practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution
