Key Responsibilities:

Complete annual risk assessments during 45-minute home visits, including annual health or post-discharge assessments Evaluate patients prior and current medical histories, medical conditions, and medications Educate members about their health conditions and medication regimens Identify care gaps and provide actionable recommendations; treatment planning and medication prescribing are not required

Qualifications:

Active and unrestricted Nurse Practitioner license in Washington Board Certification through AANP or ANCC Current BLS/CPR certification Minimum of 6 months experience as a Nurse Practitioner preferred MSN from an accredited nursing program
