Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in their homes Review and evaluate current and prior medical history, medical conditions, and medication regimens Educate patients on their health conditions and prescribed medications Identify care gaps and recommend steps to address them Perform post-discharge and annual health assessments (

Qualifications:

Active Nurse Practitioner license in Washington State Board Certification through AANP or ANCC Current Basic Life Support (BLS/CPR) certification MSN from an accredited school of nursing Minimum of 6 months recent experience as a Nurse Practitioner preferred Experience or interest in home health care is a plus
