Key Responsibilities:

Complete home health risk assessments for Medicare Advantage plan members Review and document prior/current medical histories, chronic conditions, and medication use Provide education to patients regarding health conditions and medication management Detect care gaps and offer recommendations to support overall health No responsibility for developing treatment plans or prescribing medications

Qualifications:

Unrestricted Nurse Practitioner license in Washington Current BLS/CPR certification Board Certification through AANP or ANCC required Master of Science in Nursing (MSN) from an accredited program At least 6 months of recent experience as an NP is preferred Compensation for this opportunity is $80$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
