Key Responsibilities:

Perform in-home annual health risk assessments that typically last 45 minutes per visit Review prior and current medical histories, medical conditions, and medication histories thoroughly Offer education about medical conditions and medication regimens to patients and families Identify potential care gaps, recommend solutions, and help coordinate timely follow-up Focus exclusively on assessmentsno treatment planning or medication prescribing required

Qualifications:
