Job Description

Overview Badger Hospice, LLC is seeking a skilled and compassionate Hospice Visit Nurse - Weekend Days to support patients, families, and care teams during evenings, nights, weekends, and holidays. This role is essential to ensuring continuity of high-quality hospice care outside of standard business hours. The ideal candidate is a confident clinician with strong assessment skills, sound clinical judgment, and a commitment to the hospice philosophy of comfort, dignity, and patient-centered care. You will serve as the primary after hours & weekend on-call contact, providing clinical guidance, coordinating care, and supporting patients and families as needs arise. Responsibilities Serve as the primary daytime weekend visit nurse, responding to patient, family, and staff needs in a timely and compassionate manner. Provide on-call support including 24-hour follow-up visits, daily visits, admissions, and death visits as needed. Assess patient conditions and implement appropriate interventions in alignment with hospice care plans. Collaborate with physicians, pharmacy, and the interdisciplinary team to manage symptoms and ensure patient comfort. Communicate changes in patient status to the care team and appropriate clinical leadership. Provide education and emotional support to patients and families regarding disease progression and end-of-life care. Coordinate daytime care needs and ensure seamless handoff to the after-hours team. Complete accurate and timely documentation in compliance with hospice regulations and organizational standards. Schedule & Coverage Requirements This is a set-schedule, every-weekend position. Coverage hours are Saturday and Sunday from 8:00AM to 5:00PM. Holidays may be required based on organizational needs. Schedule is consistent and not rotating. Qualifications Current WI Registered Nurse (RN) license in good standing. Minimum of two years of nursing experience preferred, with background in hospice, home health, med-surg, or critical care. Strong knowledge of symptom management, end-of-life care principles, and hospice regulations. Excellent clinical judgment, assessment, and documentation skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Strong communication skills to support patients, families, and staff during emotionally sensitive situations. Valid driver's license with reliable, insured transportation. Must be able to reliably travel between Brookfield and Burlington service areas to complete visits based on patient needs.
