Job Description

Overview Badger Hospice, LLC is seeking a dedicated and compassionate Hospice Registered Nurse Case Manager to join our care team. This role centers on supporting patients and families during end‑of‑life care, ensuring comfort, dignity, and high‑quality symptom management. The ideal candidate will have strong case management experience and a deep commitment to the hospice philosophy of care. You will coordinate interdisciplinary services, guide families through the hospice journey, and provide skilled nursing support in patients' homes or residential facilities. Location We are currently seeking one full-time Hospice RN Case Manager for each of the following service areas: Brookfield Burlington Responsibilities Conduct comprehensive assessments to determine patients' physical, emotional, and psychosocial needs within the hospice framework. Develop individualized hospice care plans focused on comfort, symptom control, and quality of life. Coordinate care among physicians, social workers, chaplains, CNAs, and other hospice team members. Monitor patient conditions, adjust care plans as needs change, and ensure timely interventions. Maintain accurate, compliant hospice documentation and medical records. Provide education and emotional support to patients and families regarding disease progression, hospice services, and end‑of‑life expectations. Advocate for patients to ensure they receive appropriate hospice services and resources. Participate in interdisciplinary team meetings to review cases and enhance overall care quality. Qualifications Current WI Registered Nurse (RN) license in good standing. Experience in hospice, palliative care, or case management strongly preferred. Knowledge of symptom management, end‑of‑life care principles, and hospice regulations. Strong organizational skills with the ability to manage multiple hospice cases. Excellent communication skills for supporting patients and families during emotionally challenging times. Familiarity with hospice EMR systems and documentation standards. Ability to perform thorough assessments and deliver compassionate, patient‑centered care.
