  • Kathryn, Hospice Registered Nurse About the role A Wisconsin licensed Registered Nurse who is employed to identify, provide for and accept responsibility for the fulfillment of patient's total nursing needs.
This includes all nurse practice acts such as assessment, medication management, documentation, supervision of other nursing personnel and performing as the care manager. Essential Job Functions Maintains and actively promotes effectivemunication with all individuals. Maintains a positive image of the entity in themunity keeping in alignment with our mission, vision, and values. Responsible for on-call services, as assigned. Participate as part of the core team providing care and services and be designated to coordinate the implementation of the plan of care. Actively participate in coordination of all aspects of patient's care in accordance with current professional standards and practice. Provide education and counseling to the patient and, as needed, to the patient's family, consistent with the plan of care. Actively participate in Health & Hospice sponsored in-service training, and continuing education opportunities. Complete all documentation in time frame determined by policy, including ongoing observation and assessment of patient / family needs. Lead, guide, and direct others in the provision of quality care. Practice infection control mitigation at all times. Maintain honest, open, and professionalmunication with others including promoting the essence of the interdisciplinary team. Respond by phone to patient / family calls in a timely manner. Performs other duties, as assigned. Requirements Commitment to quality oues and services for all individuals. Ability to relate well to all individuals. Ability to maintain and protect the confidentiality of information. Ability to exercise independent judgment and make sound decisions. Ability to adapt to change. Experience & Training Must be currently licensed as a Registered Nurse in the State of Wisconsin. At least 2 years of experience in Medical Surgical, Health, Hospice, or Long-Term care nursing preferred. Must be sensitive to the need of older adults. Certified in Gerontology, Medical Coding, Wound Care (WCC), Hospice and Palliative Nurse preferred. Schedule Monday to
Friday:
8:00 AM to 4:30 PM 0.8 FTE (32 hours a week), or 1.0 (40 hours a week) On-Call Requirements This role includes on-call coverage, with additional incentives provided. Up to 1 week from Monday to Friday, 4:00 PM to 8:00 AM Up to 1 weekend, every 4th weekend, 24-hour Pay $40.43 to $47.04 per hour, based on experience (Higher pay may be considered depending on qualifications and experience) Location Hit the road without the extra costs—this role includes travel within a one-hour radius, and apany car is provided for employees working 0.6 FTE or more.
